Welcome
ConnectShareCare is a welcoming, safe space where you can connect with others. We are supported by The Dartmouth Health System. Your information is not used to generate funding, shared, or sold to third parties.
ConnectShareCare is a secure forum and is moderated by an expert facilitator. This means only members (people who have created an account) can see who is in the group and what they post. Please see the Privacy Policy for more information on how to protect your privacy.
THE IDEA
ConnectShareCare was designed by a group of patients, care partners, clinicians, and health system researchers. The network began as a resource for active and bereaved care partners of people with serious illnesses. We are now adding support groups that address the needs of people with other conditions.
FUNCTIONS
There are two main functions at ConnectShareCare:
- Connecting with others for information
- Facilitating emotional support
To achieve these functions, the following features are utilized:
- An intuitive user interface
- Data security
- A supportive and respectful space
- Incorporating and helping facilitate connections
- Evaluating and providing access to trusted resources
- Reducing the difficulty of asking for help
- Creating lists of frequently asked questions
- Identifying the everyday needs of members
- Ensuring security through moderation by facilitators
